What is the shelf life for an opened bottle of maple syrup? Does it need to be refrigerated?
The last bottle I opened I put in the refrigerator. Since my wife and I don't use it up very fast it developed a layer of crystalized sugar on the bottom of the jar. I tried heating the bottle up in the microwave and soaking it in hot water. Neither of these methods would melt the sugar that formed on the bottom of the bottle.

Typically it's a sign of being overboiled but you'll be fine. Takes a lot to spoil real syrup

Yeah, that's what a lot of guys call sugar sand, not a sign of spoilage at all.

Sounds like it was over density (over 66..8 or .9 brix) The excess sugar as come out of solution. It's fine. if it does mold on you, pour all into a pan/pot, bring to a boil, skim off crud on top and put in clean container.

We put on our labels that it should be refrigerated after opening. As to crystals forming on the bottom of the bottles, I have seen some of that in other producers at times but in my 6 years I have not and we store some for up to 18-24 months at times. It is water soluble however so you could pour the good syrup in a container and put a small amount of water in the bottom and let it sit for a bit if you did not want the crystals in the bottle or jar.
There are no preservatives or additives in maple syrup and thus mold can be formed over time.
