loin cloth needs no pattern

you have your hide there fold it into approximately 1/3s bringing the outside edges in .
the finished folded piece should be about as wide as the inside of your hip bones now sit/lay on it so you have equal front and back bring those ends up your back and up your front , now tie a sash around your waste and fold the ends back down , arrange the hide at the sash a bit spreading it out to cover hiip bones and there you go a loin cloth.

a set of leggings also secured around the waste and a long shirt and you are good to go

Hollywood would have you believe it was more like a flap that lifts up while it is actually a lot more like a cloth diaper on a baby secured with a sash rather than pins

GCP is correct, loin cloths (or breechloths) are simple. I find cloth more comfortable. Most of mine are generally 12-13 inches wide and 5.5 to 6 feet long. That's a bit bigger than most of the Eastern woodlands ones were in the 18th century, but a bit more modest. Tie something around your waist, I use a large silk square. Pull the front flap up through the sash and drape it down to just over your knee caps. Pull the rear flap up through the back and let it drape down the back of you legs. All done.

With a long shirt and leggings you are generally good, though there will still be a fair amount of cheek flash when walking and especially when sitting.

Is there a fold or something that’ll keep stuff from falling out the sides?

The extra width forms a pouch of sorts that keeps the boys in place. Just make sure to keep the cloth pulled up snug in the belt.

go wider with the fabric and fold to the middle inside from each side .

once it is on you can pull it wider at the belt

I can't say I ever tried to play LaCross in one but falling out didn't seem to be much of an issue just walking around

I've played a lot of lacrosse and other rough and tumble games and have never had an issue. Just make sure whatever sash/belt you have around your waist is good and snug.
