Sample contract forms

Hi guys,
I am wondering if anyone has a sample of a contract they use with customers. I am a part time NWCO but am realizing it would be best to have things in writing to avoid surprises or issues once I start a job. If anyone has a copy of one (picture, pdf, word doc, etc) and are willing to share it with me I would really appreciate it! Thanks.

Different states have different laws. Which require different wording in contracts.

I am in New York. I am just curious to see what sorts of things different NWCOs address in their contract (for instance, charging or not charging for non target animals, non refundable set up fees, length of contract, etc) so I can see what sorts of things I should include and which are not as crucial.
