Just another reminder: I need your name ( Very few people have trouble with that one ) I need your address ( If you're having trouble, go out and look at what it says on your mailbox ) that should be followed by the city you live in ( You would be absolutely amazed at the number of customers who don't know where they live ) And the state and zip code.
