2005 Honda Civic 4 cyl. Any idea what would cause the spark “plug” to disintegrate? Left debris on top of cylinder

Hard to tell from the leftovers and only a couple pictures , but I'll take a guess.
Looks a bit like your coil pack was arcing all over the place. Not sending the spark where it needed to be like through the spark plug to it's electrode. This over time can overheat/melt things and it gets worse as the problem goes on.
Do you see lots of carbon tracks in the area?

Q#1 Do you still have a spark plug or part of it in the cylinder head ? Q#2 Is the debris contained to the valve cover/top of the cylinder head? Q#3 Does your engine have a small tube type thing surrounding the spark plug isolated from the valve train? Q#4 How was the engine running just before this? Did it feel like only 3 out of 4 cylinders were firing?

How many miles on it? Could just be died of old age and use.

Did it feel like only 3 out of 4 cylinders were firing? 1-plug is in pieces 2-looks like in valve cover 3-I think so 4-daughter said is sounded like a “flapping” noise when giving gas. Engine light came on and was flashing then it died Engine was replaced and only had 70,000 on it when it was installed last year

#1 , You need to get all the debris out of the spark plug 'tube' . Use a vaccum and /or compressed air. Once you get it all out , you may find the base of the spark plug still there. It is important to clear it all out because when you remove the base of the plug , anything left will drop into the cylinder head - very bad.
Then inspect the spark plug 'tube' for damage. If it is in good shape , you are golden , no junk got onto the cylinder head / valve train. Check also if there is any signs of oil seepage there too.
You will obviously need a new coil pack and spark plug. Best practices , replace all of them. If one goes , the others are about due. Buying online for a set of coil packs will save you some bucks if you can wait. Look for decent replacement ones , don't go with the cheapest brand X.
Do these repairs , reset the CEL ( check engine light ) and you are done.

I may be wrong but it looks to me like you have more problems than coil packs and spark plugs. The long piece on the right hand side looks like a broken valve stem. Top of it appears to have machined grooves for the retainers. Top left looks like the stem side of a broken valve head. The "flapping" noise might have been all the loose stuff beating the crap out of the piston and cylinder head.

Yeah, I see that now. Fixin to take it to someone to look at it. Pull all the debris into an empty vacuum cleaner , then inspect what you have , be it spark plug/coil pack stuff or valve train stuff. If it is valve train stuff , there will be oil with it. Your picture looks dry.
