Both my storm doors swing out on my house and back shed and never had a problem. And I live in a heavy lake effect snow belt. Entry/Exit ways here are built with wind/weather direction in mind as drifting of snow can be a Hugh problem. Seeing as his doorway had no room to have the door swing in the easiest solution was to have the door swing out if you put in a full step. A lot more work would be needed to cut the floor back to accommodate an inward swinging door.

A lot of people refer to these as cobblestones but they're more properly known as setts. They quarried these from the late 1800s through the early 1900s and they were used to pave city streets before asphalt came along. That's a pretty good representative of the size they were striving for. 8-10" wide by 10-12" long by 4-6" thick. They can vary quite a bit and are a pain to put in place.
