Well the title pretty much says it all, constantly falls off my belt and pulls belt out of my belt loop.
This has been going on for years and I think I finally need to address it lol

Dont just clip it onto your belt, clip it over your belt and your pants. If its a loop, then just loop the end of your belt through it so it can't do that.

Or just wear it on your belt buckle side... then you belt can't pull through and that wont happen.

Buckle the belt on the last loop where you can still drop your drawers down but the belt is still hooked in a loop

I swtiched to wearing a tactical belt years ago. It goes overtop a velcro belt normally but lately i just buckle it over a regular belt. The tactical belt is stiff so it doesnt sag. My multitools live on that belt when not in use. The tactical belt is great for the bathroom trips, driving or just relaxing at home as you can pop the belt off easily when you want to loose it.

Cant get a photo to load but my gerber and streamlight are on a blackhawk paddle. The gerbar is in a double stack mag pouch. It goes over belt and britches and doesnt slide around.

course you could always flip your belt around so the buckle is on the side of your multitool.

unbuckle , then rebuckle with a new hole you make as far out on the belt as you can then nothing can slide off but you can get your pants around your ankles

better holsters are also an option , pancake style Leatherman holster https://www.etsy.com/listing/506226189/open-top-canted-pancake-multi-tool
because the pancake grabs the belt in 2 spots it slides less.
