I'll bite. In all honesty there are too many variables and there is no "one shoe fits all". Various states may or may not require certain verbage. Basically YOU need to determine what your terms are and they need to outline the service(s) that you will be performing as well as what the customer is agreeing to pay for these services. You need to include a descriptive term of the service(s), materials, specific location, start and end dates, fees that may include set up, additional labor, trip charge, trap loss fees, mileage, tax, per capture fee or all inclusive fee, warranty, guarantees, payment terms, late fees, returned check fees, etc. Keep in mind that a reasonable amount of research is required on your part. Others on this forum may have consulted with attorneys, CPAs, printing companies, etc., at their expense and may not be willing to just give away this info. My service agreement has evolved over the last 4 years and I finally have a one page document that suits me, protects my interests and is legally enforceable. You might create an outline of what you want and then consult with an attorney as a few hundred bucks may save you a lot of headaches. Good luck.
