Ended up having to get rabies shots Friday evening. Still have to get a shot on day 3, 7, and 14.

Going forward, now that I will be vaccinated, what would I do if I go bit, scratched, or cut again?

I asked the PA who gave my the shots and she didn't know.

You will still need a shot just not the whole series. But you should also have a titer count done, annually I think, to monitor your antibody level to see that it is still effective.

Typically each case is different. If the animal was tested and proved positive or not. Or is it was not available for testing. If tested positive it may require a single follow up injection depends upon the physician. Also how long has it been since your last treatment?

Your immune system will develop anti bodies from the initial treatment series.

Your health care provider may have you get your Titer count checked and to determine your levels of antibody protection. This is done with a blood test.

Most physicians will err on the safe side and give you a booster which is typically based upon your body weight.

I have had three separate treatments in the last 30 years. The most recent about 5 years ago. I get frequent scratches and bites, just part of my life style. I guess I am still ok.

Antibodies circulating in your blood stream give you some peace of mind when it comes to rabies protection for a trapper or those that handle wildlife or have frequent exposure to them.

You will most likely get varying opinions on post treatment protection from an average physicians thoughts.

I've had the Shots

The titer test at 4 and 6 years after

Booster SIngle shot at 6 years

* if you get scratched deep or bit you can get a single shot.

Most doctors are not very knowledgeable about the particulars of rabies treatments, especially when it comes to pre exposure shots, titer levels and what to do if your titer is low. I have found that veterinarians are far more knowledgeable on this subject as they all have had the pre exposure shots and need to monitor their titer levels and take the necessary action when the titer is low or when they get exposed. Doctors who have practiced in 3rd world countries where rabies is more common also have a better understanding than the average doctor in the US.

I had the pre exposure series. Had my titer checked a couple years later and my immunity was so low, it was not detectable by the test. My DR did not have a clue but I had the opportunity to talk to the state veterinarian. She said I was only the 2nd person she knew of that had had my result and that I needed to have a booster and then retest a few months later. I got the booster and six months later my titer test still indicated my immunity was below the recommended level but it was detectable now. Not sure what the next step is at this point. Probably another booster.

On the other hand, I know folks who have the pre exposure series 20 to 25 years ago and their titer is still at the recommended level.

Thanks for the great info.

I have asked doctors in the past about getting the pre-exposure shot, because I knew this day would come at some point. They have always balked at the idea.

Bob, who do you go through to get your blood tests and boosters?

My blood tests were handled through my primary care doctor and my primary care doctor referred me to another clinic to schedule my booster shot after I explained what the state vet had told me. But my primary had no clue what to do based on the results from the test.

Very True about the Local Dr.s and clinics knowledge

My Dr. Did his homework and pushed their pharmacist to help.
But initially their knowledge was unbelievably poor.

We are blessed in the US to have eradicated or reduced many diseases to the point that most mainline docs have little to no experience with them.

A decade or so ago I was called in to trap flying squirrels in a pole yard office after a couple guys contracted murine typhus from the fleas fliers carry. The first guy very nearly died as the docs were treating him for flu. At the last minute they called in an infectious disease expert versed in third world disease and he recognized typhus immediately.

Ever since then I've tried to bone up on just what I might be exposed to and am sure to let any docs I might go see know. Back when we went through a brain tumor scare, turned out to be a benign congenital thing, with our youngest I had her screened for all possible zoonotics.

It also applies to veterinarian as well. Ask any of these young vets if they've ever seen an active case of canine distemper and odds are they'll say no.

Pretty much all of them. Skunks, raccoon and bats are the top of the list for being rabies vectors but it could be almost anything we trap.

In my area, raccoons, fox, and skunks seem to be the main culprits. I will see signs every year that state rabies being found in that area.

I was talking to my doctor about the rate of rabies in bats. He said that if you woke up one morning and there was a bat flying around your room, you should get rabies shots ASAP.

Bats are actually a low risk, I've never had one come back as a positive. They don't carry it for long like skunks do. It usually kills them quick. BUT, with something like rabies you always act as if and don't takes chances.

Warrior---do you do anything special when handling skunks?

I am ate up with them in my part of the state.

Since I just trap my properly, I remove them from the trap (after they are dispatched), place a burn barrel over them and incinerate them on the spot. Never have to lay hands on them.

Not really, dispatch and dispose. The virus doesn't survive at room temp and I bag and drop into the compactor at my local transfer station.

I joke with the crew there that they are where I go to hide the bodies.

The rabies pre exposure shot series is pretty expensive, but Veterans registered In the VAMC can get them, titer tests and boosters free or at very low price. You may have to argue with them and/or go see the Vet Rep, and or the Patient's Advocate, tell them you handle wild animals and NEED them.

Got my titers checked approx. 8-10 years ago... they were STILL good from my original 2 pre-exposure shots 35 years ago. My doctor called my titer check " possible exposure" and my insurance covered it.

I still carry my pre exposure immunization certificate card from 1983.

I received 3 doses of 0.1ml intradermal doses of Imovax rabies vaccine. 11/30/83 39 years ago. My Titer is above avg. I am told.

A local family practice doctor offered a clinic to all PTA active members of District 3 or those actively involved in the ADC businesss. It was a good program for those that benefited and took advantage of the opportunity. I was doing lots of state hoping back in those years for fox and coyote trapping.

Dr. Montagna was the physician who I got to present this program back in those days.

He was very knowledgable in some areas other physicians had no experience or training. He died several years ago while mowing his lawn.

He was also very well schooled in nutrition and the benefits of grape seed extract.

Your PCP should be able to order a script for a Titer level number when needed.

My last treatment series was done as an out patient. The ER doc gave me my first treatment there.

They generally inject directly into the bite or initial exposure area for the first series. At least that is what they did the last time.

I received one immunoglobulin injection in my hand at the location of the cut. Then I received 8 more shots. 1 rabies vax, 6 immunoglobulin, and 1 tetanus.

I have the pre exposure shots. A few years ago I was bit by a red fox that escaped. One booster shot took care of everything.
