Since having the adverse reaction to the vaccines, I've seen many different doctors, 9 so far and have not had to fill out anything like that.

I don't know. I have an abscessed tooth that needs some attention and my dentist retired. No one else locally could see me so I went to the clinic to get an antibiotic prescription.

I've seen that on a doctor's form before. I wrote NONE. As in none of their business.

I had a multitude page document I had to fill out once at a doctor's office. Last page was all financial information like where do you work? How much do you make? Name of your bank? Do you own a house? Do you own stocks etc.
That IMO had nothing to do with why I needed to see the doctor. Just for the doctor to find money. That page got ripped off and thrown out.

Bonus tip - if they ask what your hobbies are put down 'reading' . Almost any other hobbies will have some kind of risk factor or reveal some kind of personal information that can be used against you.

Yep I would check every box. If questioned I would tell them how stupid the questions are. Any doubts about gender after that...I'm proud, I'll show them!

especially the lower seacoast and its a shame that its trending that way.

The Doctor just wants to know if you are mentally ill, and selecting one of the non-genders is an easy way to find out.

Been that way for quite a while. The northern part of the state, Bangor on up, used to be very conservative. Not anymore.

I guess some of the questions they asked could be used as a mental health screening. They're all nuts as far as I'm concerned.

You actually shouldn't fill those out, unless you are getting paid to do it. They are selling all of your answers after they get it from you to big corporations that will be used to target people like you. There is also always the possibility that the government buys the same data packages, but who really knows?
