Way back years ago I used to check the box (Other) on forms asking for race. That was on forms that ask about race. Didn't have White as a choice said. Caucasian.. Well at that time doppy me didn't know I was supposed to be Caucasian. Knew I wasn't Black, knew I wasn't native American, Asian or pacific Islander. So there for I must be (Other).
Dang if I can recall anyone ever asking me about checking that box. Have done it many times even after knew what Caucasian meant. I consider myself American of German/Polish decent there for if it's not listed. Must be (Other).

One last times I was at clinic different department than I been before. I got handed one the forms like on the first post. Guess Im a weirdo because I didn't have problem figuring out filling it out. It didn't have any questions about guns in my house. So I made a point of ask if they wanted to know. Told me hadn't ever heard of that question on a hospital form. Told them heard it's out there.
I do know I must filled the form out right because got me the right doctor. Didn't end up seeing a gynecologist for my tooth problem was sure enough Oral surgeon.

It's such a rough world every one wanting to get me.. The dang paper work is brutal. LMAO!
