I am 99.9% sure its a scam. Time to call S.S. Part B is deducted from your benefit. You are given the option of signing up for it or declining. Part A is part of your benefit.

I actually had to retire sign up at a social office to start my medicare retired at 63 at 65 my A&B started automatically despite receiving 6 million letters from ss it was automatic

When I turned 65 I was required to sign up for part A, everybody is. It’s free but you have to sign up for it. You are not required to sign up for part B if you can provide proof that you have other comparable health insurance. Part B is not free.
I provided the required proof that I had alternate insurance and did not have to sign up for part B.
Eh...wot?

My understanding is you never have to sign up for Part B. The penalty only applies if you ever sign up for part B and it just means it will be more expensive when you sign up to make up for the money the government didn't collect monthly payments for all those months because you chose not to start when they wanted you to.

You generally do not have to sign up for Medicare Part B at 65 if you have "creditable" coverage through a current employer (yours or a spouse's) with 20 or more employees, as you can delay enrollment without penalty.
This is what I did. I don’t think I’d face any penalties if I ever signed up for part B, which I will probably never do.

She won't turn 65 until April 25, is still working and does have health insurance through the hospital she works at. Her financial advisor signed her up for SS and her SS will go into her retirement fund with the financial advisor, then they pay her a set amount monthly out of her retirement fund. Seems to me they're jumping the gun since this is due March 25 and she isn't retiring until April 31.
