So do you have to ask for a pardon or can a President make you look guilty by giving you one even though you didn’t ask for one?

You can ask, or the president can just issue you one. Sometimes people don't accept the pardon, because doing so implies guilt and they want to maintain their claim of innocence or because they feel like they don't deserve it. A couple death-row inmates rejected commutations from biden because they want to prove themselves innocent on court. A few J6ers rejected Trump's pardons because they felt like they didn't deserve to be pardoned.

There is a J6er in NM that refused it. His charges are misdemeanors and he wants his day in court.
